  • Client Data Reporting/Service Level Reporting - OASAS requires all OASAS-certified chemical dependence treatment programs and approved gambling outpatient programs to complete an on-line admission form (PAS 44N and PAS 44G respectively) and an online discharge form (PAS 45N and PAS 45G respectively) for each client that include detailed information for non-crisis services and a combined Admission/Discharge form (PAS 46N) for crisis services that contain detailed demographic, referral and treatment information.
  • County Planning - OASAS also maintains a third password protected web-based interface for providers and counties called the County Planning system that contains numerous county and statewide level reports.
